Disorder-specific psychotherapy for persistent depression: evaluation of Cognitive Behavioral Analysis System of Psychotherapy (CBASP) in group setting

Introductie
Cognitive Behavioral Analysis System of Psychotherapy (CBASP) has shown to be an effective disorder-specific treatment for persistent depressive disorder. However, evidence for effectiveness of group treatment (Group-CBASP) is limited. Therefore, our aim was to evaluate the efficacy of Group-CBASP. Based on literature, we examined age of depression-onset as a potential moderator.
Materiaal en methodes
We conducted a retrospective study (November 2011-March 2017) evaluating treatment efficacy in 54 patients (29 late-onset, 25 early-onset). All patients were previously treated for depression but did not achieve long-term recovery. Group-CBASP involved 24 weekly group sessions and was followed by 6 months of individual CBASP. We evaluated outcomes on the Inventory of Depressive Symptoms – Self-Report (IDS-SR) at baseline and after 3, 6, and 12 months.
Resultaten
Mean IDS-SR scores decreased from 39.83 (SD 9.42) to 33.78 (SD 14.61; t = 3.62, p =.001) after 24 group sessions. Effect size was nearly medium (Cohen’s d=0.49). The late-onset group showed a larger effect size (d=0.62) than the early-onset group (d=0.36).
Conclusie
Severity of depressive symptoms declined from severe to moderate after completion of Group-CBASP, with effect sizes comparable to individual CBASP. In contrast to our expectations, late-onset was correlated with better treatment response, which may be explained by higher rates of comorbidity and single family status in early-onset patients.
